Exocrine Pancreatic Insufficiency

The pancreas provides enzymes to help in the digestion of food, further breaking down carbohydrates, proteins and fats that are eaten. When the pancreas does not make a sufficient amount of the enzymes needed to support digestion, a clinical condition called exocrine pancreatic insufficiency (EPI) can result, where a person can experience loss of weight, bloating, abdominal pain, and foul smelling, bulky stools due to the malabsorption of fat. In addition, individuals with EPI have difficulty absorbing the fat-soluble vitamins A, D, E and K.

The symptoms of EPI can be managed by taking a pancreatic enzyme containing lipase, to support fat digestion; protease, to support protein digestion; and amylase to support carbohydrate digestion. In addition, patients may need multivitamins with higher amounts of the fat-soluble vitamins than is commonly found with many over-the-counter (OTC) brands to supplement their diet, and often require mineral supplementation (e.g. Iron, Magnesium, Selenium, Copper, Manganese and Chromium).

Steatorrhea (Fat in Stools)

Gas, abdominal pain, bloating can all be signs that you have enzyme de­ficiency, enzymes normal secreted by the pancreas and enzymes essential to the proper digestion of food. When food is not property digested, you may also see low levels of the vitamins A, D, E, and K; and certain minerals. 

Malabsorption of Fat

Chronic diarrhea and excessive weight loss are the most common symptoms of the malabsorption of fats, however, they are not speci­fic to EPI. Other common symptoms include abdominal pain and bloating, varying from constant discomfort to severe abdominal pain. Other clinical manifestations are directly related to malabsorbed nutrients, where diet alone does not provide sufficient amounts. Due to the lack of fat assimilation, individuals often cannot benefit from fat-soluble vitamin intake and suffer from low vitamin levels (i.e. hypovitaminosis).
